“Band-Aid” Medical
Reimbursement Plan
Inexpensive alternative to a full
medical insurance plan, or can be combined with major medical only
plans. This policy will reimburse for 2 to 4 doctor’s visits per
year. Several levels and choices available.
Group Dental Insurance
There are two plans available: a
DHMO* and a PPO*. Coverage available for employee only, or
employee and their dependents. Rates require at least a 50%
contribution of premium by employer. Online internet support
allowing you to change dentists and obtain other information.
Illness, Injury & Maternity
Disability Insurance
Think of this as paycheck
insurance/income replacement. If you are unable to work due to a
non-work-related injury or illness, this policy pays you 60% of
your normal gross income. At age 35, a person has a 50% chance of
being unable to work for more than three months due to a
disability before age 65. This coverage is very affordable and is
a wonderful employee benefit. 100% participation of employees is
required based on either 20 hours per week or 30 hours per week at
the employer’s discretion. Benefits begin after two weeks off
work and are payable until age 67. This is short term and long
term disability coverage. A group term life insurance policy with
$10,000 coverage is also available.
Retirement Savings Plan
Individuals may begin today their
participation in the incredible retirement program available to
non-profit organizations—the 403(b) plan. The American Funds*
are providing the mutual fund program to assist each individual
with their own retirement. This program does not require employer
participation. Each employee may contribute 100% of their income,
up to $12,000 in 2003. There is a nominal $10 set up fee to open a
new account, with no additional fees charged in the future. The
minimum opening payroll deduction is $25.00 per fund.

is administered through SmithBarney Citigroup in Pasadena,
California.
Student Accident Insurance
Students and adult volunteers
are covered:
- On their way to school;
- While at school or a
school-sponsored event;
- On their way home from school
In today’s litigious society, any
non-profit organization is at risk for lawsuits stemming from
accidents and injuries. Here is a way to help reduce this risk.
This policy will pay up to $500,000 for medical and dental bills.
Included is a $10,000 death benefit. This is called a “full
excess” policy, meaning if there is primary insurance, this
policy will pay deductibles or co-pays. If there is no primary
insurance, this will drop down and become the primary policy.
There is no deductible. The minimum annual premium for ELEA

Organization —coverage is for dentists contracted on this
panel and are selected from a company list.
PPO: Preferred Provider
Organization—The Dental PPO allows employees to choose between
using a network dentist or visiting
any dentist. When using a network dentist, they enjoy a greater
reduction in cost.
